A written, signed and dated order from a physician must be provided to the supplier before the claim is submitted to Medicare. Home-delivered catheters and accessories are covered when all of the following criteria are met:

  • The person must be eligible with an expressed written order from their doctor for home-delivered catheters and accessories.
  • The person must meet all other applicable Medicare statutory and regulatory requirements.

PRIVATE INSURANCE

Most but not all private insurance companies follow Medicare guidelines for home-delivered catheters and accessories qualification requirements.
